Your pharmacy benefit includes a mail order option that allows you to obtain medications that you take on an ongoing basis from your mail service pharmacy.

Getting Started

Mail Service Order Form
This form should accompany your initial prescription order, or you may choose to enroll online below.  You can print and mail this form along with your new prescription.  After enrolling, your physician may phone or fax in your prescription (see link below).
