Do you need to contact a customer service representative?
For inquiries related to your pharmacy benefit, please contact the customer contact center.  The Customer Contact Center is available 24 hours a day, 7 days a week, 365 days per year.  Please call 888-648-6759.  The interactive voice response (IVR) system can provide you with self-service answers to many of your questions, or you may speak with a customer service representative.

Direct Member Reimbursement
If you purchased a covered medication without the use of your pharmacy benefit card or at a non-participating pharmacy, your plan sponsor may allow for direct member reimbursement. To submit a claim for direct member reimbursement, you may access and print the direct member reimbursement form below.

Once you have completed the direct reimbursement form, mail it along with your prescription(s) receipt to the address at the bottom of the form.
Approved claims are paid on a biweekly basis and a check will be mailed directly to the address that you provide on the direct reimbursement form.
